Your performance ratings of the student are important to us. In addition to providing performance-based evaluation of this student, feedback from the site supervisors helps us evaluate and improve our program.

The ratings should be based on your observation and reports received from others (i.e. staff, parents, students). In situations where a student has more than one supervisor, it is okay for (a) each supervisor to complete the surveys separately or (b) all of the supervisors to complete the surveys as a team. Choose the level of expectation from the scale that best describes the student's current performance based on the descriptions below:

  • Fails to Meet Standards: Student consistently performs poorly for their level of training and may need a Student Success plan.
  • Below Standards: Student performs below professional expectations given their level of training and requires improvement.
  • Meets Standards: Student performs at the level of professional expectations given their level of training.
  • Above Standards: Student consistently exceeds professional expectations given their level of training.
  • Not Observed: Insufficient data/opportunity or may be inappropriate given their level of training.
