M.Ed./Ed.S. in Learning Sciences Education Limitation Form

My signature below indicates that I have been informed and accept this agreement. I have been informed that:

• In earning this M.Ed. or Ed.S. in Learning Sciences Education, I will not be eligible to teach in the program subject areas as I do not hold an Illinois Professional Educator License.
• This degree may not necessarily fulfill requirements for other U.S. state licensure endorsements.
• I may be lacking in other requirements by other state licensure bodies thus making me ineligible for licensure endorsements.
• I am responsible for adhering to these requirements.
